  • identifying missing medical records
  • assembling all of the information in chronological order, usually in a verbatim manner for broadest use

Narrative Summaries
This narrative summary of the medical chronology often includes:

  • a cogent, easy-to-understand, narrative summary of the medical records and their impact on the case
  • a chronological summary of pertinent medical records
  • background information on the patient
  • healthcare providers that treated the patient and dates of service
  • injury data (related to the claim)
  • potential alternative causes of the alleged injury
  • risk factors prior to the alleged injury
  • significant medical and surgical history (including pre-existing conditions)
  • drug prescription history, if pertinent
  • instances of the patient’s non-compliance with medical care and recommendations for follow-up care
  • a recommendation of medical experts
